Documentary Requirements
1.) General Requirements
- 2 pcs 1 x 1 picture
- Marriage Contract (if married)
- Birth Certificate (if single)
- Proof of Billing (Electricity bill, Water bill, Barangay Clearance)
- Cedula latest
- VALID IDs - Government ID, Company ID w/ SSS no. and TIN no. (Photocopy)
- Pag-IBIG MSVS - Membership Status Verification Slip
2.) Membership Status Verification Slip (MSVS)
- With 2 years Pag-IBIG membership contribution
- 24 months contribution can be paid lump sum if not yet a member of Pag-IBIG
- Approved (with signature of HDMF verifier and certifier)
- Contribution must be updated
- With Employee Statement of Accumulated Value (ESAV)
- With Seaman's Passbook (If OFW)
- Multi-purpose loan (MPL) must be updated
- MSVS must be properly filled up by the borrower
- Write the Developer name and birthdate on the top of MSVS
- Contribution must be reconciled if the borrower had a new employer
3.) For POP/OFW Reservation

Considered income on Job Contract
- Basic monthly salary
- Fixed overtime
Job Contract must be or have:
- Latest
- Duration of Contract (from when and up to when, month and year stated or with number of months) indicate if unlimited contract or regular
- Company stamp
- Authorized Company Signatory
- POEA stamp (if seaman)
- Designation
SPECIAL POWER OF ATTORNEY
Executed before departure
- SPA must be Notarized
- Originally signed by the Principal Buyer and Attorney-in-Fact
- Complete data/information
When Buyer is on Abroad
- SPA must be authenticated by the Consul
- Originally signed by the Principal Buyer and Attorney-in-Fact
- Complete data/information
